  • Premature Return-to-Play Clearance: Relying on flawed baseline data increases the probability of clearing an athlete to return to contact sports before their brain has fully recovered.

  • Heightened Risk of Severe Injury: Returning to competition while still concussed exposes the athlete to catastrophic conditions such as Second Impact Syndrome (SIS), prolonged post-concussion syndrome, and cumulative neurological damage.

Establishing Best Practices for Reliable Baseline Testing

To ensure that baseline assessments provide a trustworthy clinical benchmark, testing environments must be strictly controlled and standardized:

  • Private or Low-Density Testing Environments: Administering baseline tests individually or in small, tightly supervised groups where noise and visual distractions are minimized.

  • Direct Clinical Supervision: Trained test administrators must actively monitor examinees to verify comprehension of instructions, enforce quiet, and identify signs of poor effort or fatigue.

  • Pre-Test Screening: Evaluating the athlete’s current state—ensuring they are not sleep-deprived, acutely stressed, or sick—before initiating cognitive testing.

  • Multi-Modal Diagnostic Strategy: Utilizing computerized cognitive baseline testing as one component of a comprehensive evaluation that includes Vestibular/Ocular Motor Screening (VOMS) and balance testing.
